Initial Evaluation
It is extremely important that all pertinent medical records be made available to us prior to the time of visit. This request includes but it not limited to X-Rays, CT Scans, MRI,
Thermograms, Nerve Conduction Studies, and other physician evaluations.

Some individuals will require additional testing and or consultations before a treatment plan can be established and implemented. Some of the consultations may be from one of the following;
psychologists, psychiatrists, urologists, neurosurgeons, physical and occupational therapists, orthopedic surgeons, rheumatologists, oncologists,
dermatologists, and radiologists.
